This 25 page self study guide has been designed to aid in the education and training of professionals who are assisting clients in making decisions surrounding the need for permanency planning and, specifically, standby guardianship. Each of the six chapters outlines important areas of concern and learning objectives. A case study is used to illustrate the progression of a fictional case through the process of creating a standby guardianship.     Power of Attorney
PDF   District of Columbia Power of Attorney for Health Care Form and Instructions [from Partnership for Caring] This form is used in the District of Columbia to name an adult as your attorney in fact the power to make health care decisions for you if you cannot make the decisions for yourself. To execute a Power of Attorney for Health Care, the law requires that you sign it in the presence of two adult witnesses. The instructions attached to this document (which also contains a Living Will/Advance Directive) explain how to execute these documents and what to do with them after completed.
     
PDF   District of Columbia Living Will/Advance Directives and Instructions [from Partnership for Caring] This form is used in the District of Columbia express what health care treatment a person does or does not want if he/she is not able to make his/her wishes known. To execute a Living Will/Advance Directive, the law requires that you sign it in the presence of two adult witnesses.     Medical Consent Authorization
PDF   District of Columbia Medical Consent Authorization Form This form is used in the District of Columbia to name an adult who can consent for medical treatment for a child. To execute a Medical Consent for a minor child, use this form to name the person chosen to provide consent. A copy of this form should be given to the person designated, the child's health care provider, as well as case manager and attorney, if applicable. The form does not need to be witnessed and may be revoked at any time.
     
   

Standby Guardianship

    These sample Standby Guardianship documents have been designed by the D.C. Bar Pro Bono Program's Family Law Repres entation Committee to help people without lawyers in the D.C. Family Court. These documents have been posted to the D.C. Bar's Web site and are presented as a service to its members and the public. As stated by the Bar, although it attempts to keep this information current and accurate, it makes no warranty or guarantee that it is correct, complete or up-to-date.   The District of Columbia Bar Information and resources on finding an attorney in the District of Columbia, including the D.C. Bar Free Legal Advice Clinic which is held on the second Saturday of every month at two locations (details can be found under Legal Service Sources page).
     
    The District of Columbia Bar Pro Bono Program Pro Se Family Law Pleadings are designed to help individuals without lawyers in the D.C. Family Court. They can be used in court cases related to divorces, child custody, paternity, child support or standby guardianship. These are free materials and are provided in pdf format.
